Terms of Service & Medical Disclaimer

The rules for using T-MedInfo, posting reviews, and relying on directory information

Last updated: September 30, 2026

MEDICAL EMERGENCY: If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use this website. If you are in crisis or thinking about suicide, call or text 988 (Suicide & Crisis Lifeline).

These Terms of Service (“Terms”) govern your use of t-medinfo.com (the “Site”), operated by T-MedInfo (“T-MedInfo”, “we”, “us”). By using the Site, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Site. Contact: support@true-medinfo.com.

1. Medical disclaimer: not medical advice

The Site is an information directory. Provider listings, specialty descriptions, Medicare statistics, Patient Guides, and reviews are provided for general information only. They are not medical advice, diagnosis, or treatment, and they are not a recommendation of any provider, treatment, drug, or product.

Always ask a doctor or another qualified health professional about a medical condition or treatment. Never ignore professional medical advice or delay getting care because of something you read on the Site.

2. No doctor-patient relationship, referral, or endorsement

T-MedInfo is an independent website. It is not affiliated with CMS or any other government agency, and it is not a referral service. Using the Site does not create a doctor-patient relationship with us or with any listed provider. A listing is not an endorsement. We do not check any provider’s current license, board certification, disciplinary history, insurance participation, or quality of care. Before choosing a provider, confirm these directly with the provider, your health plan, and your state licensing board.

3. Directory data is provided “as is”

Directory records come from public datasets published by the Centers for Medicare & Medicaid Services (CMS), including the National Plan and Provider Enumeration System (NPPES) NPI Registry, Medicare datasets, and Open Payments. These datasets are self-reported or administrative records. They may be incomplete, outdated, or wrong, and our copy may lag behind the source. Medicare statistics describe Medicare claims only and do not measure quality of care. Open Payments data shows payments reported by manufacturers; a payment does not mean that anyone did anything wrong. How providers can correct or hide their information is explained in Section 9 of our Privacy Policy.

4. Eligibility and accounts

  • You must be at least 13 years old to create an account or post content. If you are under 18, you may use the Site only with the involvement of a parent or guardian.
  • Give accurate information, keep your password secure, and do not share your account. You are responsible for activity under your account. Tell us promptly at support@true-medinfo.com if you think your account has been used without your permission.
  • You can delete your account at any time on your account page. We may suspend or close accounts that break these Terms.

5. Rules for reviews and other content

“User content” means reviews, ratings, photos, and anything else you post. When you post user content, you promise that:

  • it is based on your own real experience as a patient of the provider, or as the patient’s parent, guardian, or caregiver, and it is honest;
  • you are not the provider, a relative, an employee, a business partner, or a competitor, and you were not paid or given anything of value for the review, or for a positive or negative review;
  • it does not contain anyone’s protected health information, or anyone else’s private information such as home address, phone number, or email address;
  • it does not include a photo of any other patient, or of any person who has not agreed to it;
  • it is not defamatory, threatening, harassing, hateful, obscene, or discriminatory, and it does not make false statements of fact;
  • it does not give medical advice or make claims about treatments or cures;
  • it is not advertising, spam, or links to other websites, and it is not generated in bulk or by automated means;
  • you own it or have the right to post it, and it does not infringe anyone’s copyright, trademark, privacy, or other rights.

Reviews express the opinions of their authors, not of T-MedInfo. We may review content before or after it is published. We may decline, hide, or remove content that breaks these rules or the law, without notice. We do not edit the wording of reviews, and we do not accept payment to post, remove, or change reviews. We are not required to monitor content, and we are not responsible for user content.

6. License for your content

You keep ownership of your user content. By posting it, you give T-MedInfo a non-exclusive, royalty-free, worldwide license to host, store, reproduce, display, publish, and distribute it on the Site and in connection with operating and promoting the Site. The license includes the right to make technical changes, such as resizing or converting photos. It also covers publishing it with your display name. This license ends when you delete the content or your account. The exceptions are backup copies kept for a limited time, and copies we must keep to meet a legal obligation or resolve a dispute.

7. Reporting content and copyright concerns

To report content that you believe infringes your copyright, violates your rights, or breaks these Terms, email support@true-medinfo.com with “Content complaint” in the subject line. Include:

  • the URL of the content and a description of the problem;
  • for a copyright complaint, identification of the original work and why you believe the use is unauthorized;
  • your name, email address, and whether you are the rights holder or an authorized representative;
  • confirmation that the information you provide is accurate to the best of your knowledge and that you are making the complaint in good faith.

The site operator will review the complaint and may ask for more information. Content may be removed or access restricted if it violates these Terms or applicable law. The person who posted it may be notified. If your content was removed and you believe this was a mistake, email the same address with the page URL and your explanation so the decision can be reconsidered.

This is the Site’s complaint process. T-MedInfo has no registered DMCA designated agent and does not claim the safe-harbor protection for hosted user content under 17 U.S.C. §512(c).

8. Acceptable use

T-MedInfo is not a consumer reporting agency and does not provide consumer reports under the Fair Credit Reporting Act (FCRA). Do not use the Site or its information to decide anyone’s eligibility for employment, credit, insurance, or housing, including tenant screening, or for any other purpose covered by the FCRA.

You agree not to:

  • scrape, crawl, or bulk-download the Site with automated tools, except for search engines that follow our robots.txt. You may not send requests at a rate that burdens our servers;
  • bypass or interfere with security features, including Cloudflare Turnstile checks, rate limits, or access controls;
  • mirror, frame, or republish the Site, or large parts of it, under another domain;
  • use provider contact information from the Site for unsolicited marketing, spam, or harassment;
  • impersonate any person, create fake accounts, or post fake reviews;
  • upload malware or files that are not the images they claim to be, or use the Site to break any law.

9. Intellectual property

The underlying government data is public, and we claim no ownership of it. The Site’s design, software, logos, original text (including Patient Guides), and the selection and arrangement of content belong to T-MedInfo. You may view and print pages for your personal, non-commercial use. Other uses require our written permission.

10. Third-party links and advertising

The Site contains links to other websites (such as CMS, state boards, and provider websites) and shows ads served by Google and its partners. We do not control and are not responsible for third-party websites, ads, products, or services. An ad on the Site is not an endorsement.

11. Disclaimer of warranties

The Site and all content are provided “as is” and “as available,” without warranties of any kind, express or implied. This includes warranties of accuracy, completeness, timeliness, merchantability, fitness for a particular purpose, and non-infringement. We do not promise that the Site will be uninterrupted, error-free, or free of harmful components.

12. Limitation of liability

To the fullest extent permitted by law, T-MedInfo and the people who operate it will not be liable for any indirect, incidental, special, consequential, or punitive damages. This includes damages arising from your use of the Site, reliance on any content, or decisions about your health care. Our total liability for any claim relating to the Site is limited to one hundred U.S. dollars (US$100). Some laws do not allow these limits, so they may not apply to you in full.

13. Indemnification

You agree to indemnify and hold harmless T-MedInfo and the people who operate it from claims, losses, and reasonable costs (including attorneys’ fees) caused by your user content, or by your breach of these Terms or the law.

14. Changes, termination, and general terms

We may change or discontinue any part of the Site at any time. We may update these Terms. When we do, we will change the “Last updated” date above and, for material changes, post a notice on the Site. If you keep using the Site after an update, you accept the updated Terms. If any provision is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver. These Terms and the Privacy Policy are the entire agreement between you and T-MedInfo about the Site.

15. Contact

Questions about these Terms: email support@true-medinfo.com or use our Contact Us form. Healthcare providers can send correction and opt-out requests through the Provider Data Update Request form.
